As our patient, you should be totally satisfied not only with your healthcare but with every aspect of the process. With that in mind, here are a few items to help things go smoothly when coming for an appointment with us:
First Appointment
Please arrive 30 minutes before the appointment. For your convenience, we have posted our new patient forms on this website, please complete them in advance. Please mail or bring the completed forms with you to your appointment.
Our registration forms are provided below in a pdf document format for your convenience. Please fill out, print and bring with you to your first appointment.

All Appointments
As the patient, you are responsible for bringing or delivering ALL pertinent films, for example, MRI's, CT's, and x-rays-related to your condition to each office visit. We cannot see you without them! Also, please be on time, if you are running late and do not contact us, we will need to postpone your appointment.
Surgery Appointments
You will be given detailed instructions by your neurosurgeon in advance of your surgery. If you do not understand or cannot find your pre-surgery instructions, please call our office and we will be glad to assist you.
